Hello! Below, you will find a list of forms to submit requests to the restaurant.
Once the form is completed, it will be sent to the corresponding director.
Uniform Order
Part of Chick-fil-A culture is our clean, professional appearance. To help ensure our Team Members maintain the standards, we have streamlined the process to order new and/or additional uniform items.
Vince and Meagan cover half of the cost of most uniform items. For more information, please see the provided link for photos, sizes, and, prices. For Team Member convenience, the cost of the items ordered will be deducted from the Team Member’s paycheck at the time the restaurant places the order.
Shipping time on uniform pieces can vary. When your items have arrived at the restaurant, a Leader will notify the Team Member that the items are ready for pick up.
Paid Time Off Request
One benefit offered to ALL Team Members at Chick-fil-A Rainbow and Lake Mead is Paid Time Off (PTO). We will add requested PTO hours to your bi-weekly check that is processed every other Monday. Team Members must use at least 4 hours at a time. This is DOES NOT guarantee any days off. Team Members must still request days off through Hotschedules
PTO being used for Requested Days off: Fill out the form and submit it BEFORE the requested days off to ensure it is processed for the correct days.
PTO being used for Sick Days: Fill out the form and submit it as soon as you can to ensure it is processed for the correct days.
Direct Deposit Request
If you would like to skip picking up a paper check, you can easily enroll into direct deposit. This means that instead of us printing you a paper check, your check will automatically get deposited into your bank account, savings account, or both! On payday, there will be a printable copy of your paystub emailed to you
If this is for you, print out the form, fill it out, and have a Leader place it in “Meagan’s Box”.
If you don’t have a printer, you can request a copy at the restaurant.
Pathway Access Request
If you don’t have or forgot your CFAhome login, you’re gonna need it for Pathway!
Fill out the form and we’ll send you in your login and a temporary password.
Grand Opening Trainer Request (Level: Trainer & Above)
Interested in attending a Grand Opening? Fill out this form and get the ball rolling!
Please remember, you MUST get approval from The Talent Director or Vince prior to committing to attend a grand opening.
If you agree to an opening before getting it approved and are denied the time away from the restaurant, you will be responsible for all costs associated with canceling.
This does NOT request time off on HotSchedules. Please ensure all dates are submitted and approved.
Paystub Request
Need a copy of a past paystub? You can request a emailed or paper copy today!
Fill out and submit the form provided in the link, and it will start the process.
W-2 Request
Need a copy of a past W-2?
Fill out and submit the form provided in the link, and it will start the process.
Team Members can only receive a paper copy at this time.
Address Change
It is very important we have the correct address on file for Team Members. As soon as you know your new address, fill out the form to ensure we have an up-to-date address on file.
Team Member Contact Change
Chick-fil-A Rainbow and Lake Mead’s primary way of communicating with Team Members outside the restaurant is through text message, with a quick phone call, or email. Without an accurate phone number or email address, the system of communication breaks down. So, it is important to update any changes to phone numbers or email addresses as soon as possible. Team Members can do that with the link provided.
Emergency Contact Change
Unfortunately things happen, and in the event of an emergency, we need to have the contact information for someone you trust with personal information. If the initial person you provided during orientation should be changed, please take the time to update your choice with the link provided.
Resignation Form
Life brings new adventures, and as much as we hate to see you leave us, we are excited for you!
Fill out the link provided to inform us of when your last day will be.
Fix-it Ticket
Ice machine acting up? Need a new 12qt container? Do we need a plumper? Need more 3rd party stickers? Broken Henny Penny?
